Functional Outcome of Surgical Treatment for Unstable Thoracolumbar and Lumbar Burst Fractures

Abstract
We did this project to compare the functional results according to the different treatment methods, anterior or posterior, in the treatment of the unstable thoracolumbar and lumbar burst fractures, with or without neurologic deficits. We analyzed

cases of unstable thoracolumbar and lumbar burst fractures treated surgically by anterior approach in 16 cases, and by posterior approach in 13 cases between Oct. 1988 to Aug. 1992 with minimal follow-up for 2 years and with a mean follow-up for

months. We studied in the viewpoint of change of kyphotic angle, neurologic improvement, canal clearance, complication and functional assessment. There were no significant difference in change of kyphotic angle(P>0.05), neurologic
improvement(P<0.05)
and functional results(P>0.05) in both surgically treated groups. But anterior approach resulted in marked spinal canal clearance than posterior approach (P<0.01) Anterior decompression and fusion could provide remarkable canal clearance and
ample
room
for the neurologic recovery. Anterior approach should be recommanded for the unstable thoracolumbar burst fractures with severe canal compromise and partial neurologic deficit to provide reasonable circumstance for neurologic recovery.
